Housing Application Forms

The  Housing Forms come in two types. The Word Document form can be downloaded for use with a computer. You can type into the document then print, mail or FAX the form to us. The PDF form should be printed off the web site and then you can hand write or type on the printed form.

    Once clients have completed treatment, low-cost housing is available across New York State
