Sutphin Boulevard Outpatient Clinic
The Sutphin Boulevard program located in Jamaica, New York, is a transitional housing environment and stabilization program with an OASAS licensed outpatient clinic. Our OASAS licensed supportive housing program is located nearby at 145-53 South Road, and accommodates men and women. These programs promote independence through engagement in treatment, education, training, employment, and housing.
Referring agencies include the Department of Corrections, probation and parole, treatment courts, legal aide, HRA, and detox/rehabs.
In order to be admitted, a client must be 18 years of age or older or have parental/guardian consent, must have a history of alcohol and substance abuse, and be a resident of New York State.

- Alcohol Education - one hour, 2 times per week didactic group facilitated by a counselor and supervised by a CSW. Topics may include physical effects, family dynamics, social issues, psychological issues.
- Health Education Lectures - facilitated by Registered Nurse and CSW on a weekly basis. Topics may include HIV, Hepatitis, STDs, high blood pressure, gambling, eating disorders, depression, co-dependency, heart disease & lung disease.
- Group Therapy - facilitated by counselors. Addresses personal issues relating to addiction and criminal behavior. This is an open group, and has a different focus each time.
- Relapse Prevention - daily group facilitated by qualified counselors. Gives members an opportunity to explore and share the every day problems and issues which may distract them from their recovery. Main purpose is to identify those individuals who are struggling with recovery and may need more concentrated assistance.
- Men's Focus Group - weekly group targeted toward men with a past history of abuse and domestic violence. Normally this is at least a partial result of alcohol abuse and the effects it has physically and psychologically. Twelve week track.
Family Services Program
Phase 1 (3 months) - participate in rehab module of outpatient treatment (attending treatment services daily for a minimum of 3 hours per day. Participants will participate in Relapse Prevention, Group Therapy, Individual Counseling, Health Education, Vocational Assessment & Counseling. Other activities (e.g. anger management, women's and men's issues) are incorporated as needed.
Phase 2 (3 months) - continue with outpatient treatment, scheduled on individual, as needed basis. Must complete a 12 week Parenting Skills workshop. Screening for AOD use will be done weekly.
Phase 3 (3 months) - continue outpatient treatment as needed. Must complete 12 week Family Therapy workshop (communication, issues, education, discipline without violence).
Phase 4 (3 months minimum) - successful graduates of Phases 1-3 will enter Aftercare treatment. Schedules are modified based on the needs of the individual. Determination of successful completion will be determined in Case Conference which includes the client and all applicable staff. If it is determined that the client completed the program, a note is placed in the their file and a Completion Certificate is awarded.
